New Patient Registration

Argyll_Hse.jpgWe are pleased to accept new patients who are resident in postcode areas PO32, PO33, PO34.

If you wish to register as a patient at our surgery then we need certain information from you and each member of your family so as to be able to trace your previous medical records. In order to register you need to call into the surgery and collect a new patient pack. You will need one of these per patient. You are welcome to either complete this in the surgery or take them away with you to fill in.

please note you will need photographic ID and proof of address when registering with Argyll House Surgery. 

Note that we cannot guarantee that we shall accept you as a patient since, for example, it may be that your address is not in our catchment area.   

All newly registered patients over 5 years old will be offered a health check with the healthcare assistant when they join the practice.

Medical treatment is available from the date of registration. Please contact reception for further information.
